LICENSE AND RENEWAL APPLICATION CHANGES

Effective Monday, February 14, 2011 all license applications that have a fee associated with them must be mailed to the following address:

New York, NY 10008-3817

This address is a lockbox for Wells Fargo Bank. They will be processing all of our fees as well as scanning all documents submitted with the applications.

Please note that applications submitted at either the window or mailed to the agency will be rejected and returned.

A receipt will be mailed to you as soon as the application is received by the Authority from Wells Fargo Bank.

Please remember that you must notify your municipality or community board at least 30 days before filing an application with the SLA. Should an application be received at the Lock Box before 30 days have lapsed since notifying the municipality or community board, the application will be disapproved.

Applications that do not require a fee, such as alterations and some endorsements, should continue to be mailed to the appropriate zone office.
